Step 2. After that on the home page of the website, MENU will be written on the left side, click on it

Step 3. After that Ayushman Mitra will be written below the portal, click on it

Step 5. Then a new page will open in which you have to enter your mobile number and click on “GET OTP”.

Step 6. After that enter OTP and CAPTCHA and click on LOGIN

Step 7. Then the option of download report will open in front of you, in which select your STATE NAME, DISTRICT, BLOCK TYPE, BLOCK NAME OR VILLAGE NAME and click on SEARCH

Step 8. Then a PDF will appear below it, click on it and download it

If your name is present in this list then congratulations. Because, you are eligible to make Ayushman card. You can apply for Ayushman card by visiting the nearest health center or CSC center.
